Co-hosted by the Patient-Centered Primary Care Collaborative, the conference offered a look at how advanced medical homes fit into new value-based care delivery models and spurred discussion around how it all works, what we are learning from research, what outcomes are being seen, and what issues and challenges lie ahead.

Last week, the Colorado Family Medicine residency project held its biannual learning collaborative. A partnership between HealthTeamWorks®, Colorado Association of Family Medicine Residencies (CAFMR), and the University of Colorado Department of Family Medicine, the event content was developed by the CAFMR QI workgroup. The workgroup includes members from 11 residency practices.

CPC+ is the next phase of the Comprehensive Primary Care (CPC) initiative that has been underway for nearly three years.

A new report released last week assesses the two-year results of the Comprehensive Primary Care (CPC) Initiative and shows that practices are getting better at assessing their patients’ risk of future health problems and delivered more proactive care to high-risk patients to reduce preventable readmissions and improve self-care and medication management.
