Documenting workflows, or process mapping, is a great way to capture the current need for daily and sometimes hourly changes practices are experiencing with the recent COVID-19 crisis. The advantage of mapping a process as a group of inter-related steps is that you document the process from multiple perspectives.

UNHEALTHY ALCOHOL USE – FACILITATING ALCOHOL SCREENING AND TREATMENT (FAST)

According to the Colorado Department of Public Health and Environment, one in five adults in Colorado drink excessively and five deaths per day are due to excessive drinking.  Short-term health risks include injuries, violence, alcohol poisoning, risky sexual behaviors, miscarriage, stillbirth, and fetal alcohol spectrum disorders (FDSAs) among pregnant women.

Case Study: Expanding Access to Behavioral Health

With funding from the Kaiser Permanente Community Health Fund of The Denver Foundation, Jewish Family Service has assessed and implemented plans to address significant gaps in behavioral health care for Coloradans.

International leaders from the UK, Ireland and the US are meeting for a two-day symposium in Dublin (30 April – 1 May 2018) to share best practice and push forward the transformation agenda of primary and integrated care.

The International Accountable Care Symposium is the second meeting of the International Faculty for Primary Care to look at the evidence and share experiences in delivering integrated care.
