Four of the seven regions participating in CPC – the states of Arkansas, Colorado, and Oregon, and the Greater Tulsa region in Oklahoma – realized net savings and will share in those savings with CMS.</p> <p>HealthTeamworks<sup>®</sup> is thrilled to announce that all of our Colorado practice clients that met the CPC reporting requirements also met the quality and utilization benchmarks and will jointly receive <strong>$450,086 </strong>in shared savings! Also of note, Colorado achieved the lowest all-cause hospital readmission rates of all CPC regions. We would like to thank all of our practices for their incredible work and continued participation in the CPC initiative. The results are a testament to the efforts they have made to deliver the best, most comprehensive primary care possible. We are encouraged by the results for every region as we further our commitment to support primary care.</p> <p>As a group, all CPC practices showed positive quality results, with lower than expected hospital admission and readmission rates and favorable performance on patient experience measures. CPC practices’ performance on electronic Clinical Quality Measures also exceeded national benchmarks, particularly on preventive health measures. Thank you to all the practices for traveling to the event, and to those who attended virtually. Many of our Colorado practices shared their experiences in CPC during breakout sessions on diverse topics including innovative data delivery, linkages to community-based services, and advanced primary care strategies. Mercy Family Medicine (Durango, CO) was recognized for excellence by CMS with a Champion award,  and Family Physicians of Greeley (Greeley, CO) was featured in a gallery walk highlighting their work improving patient utilization. We appreciated being able to participate in recognizing these practices’ diligence and innovative approaches in achieving CPC milestones. Our team left the conference invigorated and excited to continue improving primary care delivery.</div> <div class="mceTemp"> </div> <p>Throughout the day, CMS shared ways that CPC  practices have contributed valuable insights that have informed  innovation projects from the Center for Medicare &amp; Medicaid Innovation, including the recently announced CPC+. There was tremendous excitement about CPC+, which may include up to 20 regions and 5,000 practices.  This represents a significant expansion of the initial CPC program, which currently includes seven regions and 500 practices. CPC+ will launch in 2017, and with its highest investment in primary care to date, CMS is reinforcing the importance of primary care.  The day also included insightful  discussions around MACRA, (Medicare Access &amp; CHIP Reauthorization Act), which creates The Quality Payment Program and drives the healthcare delivery system toward value-based care in a coordinated, efficient way. CMS is  encouraging stakeholders to read the proposed rule and comment by the <strong>June 27, 2016</strong> deadline.
