Despite the urgent demand for more workers in health care, entering the workforce remains difficult, confusing, and inaccessible, especially for those facing barriers ranging from lack of employment history to insecure housing to unreliable transportation and more.Â
But these voices belongs in health care and bring valuable lived and living experience to care settings.
Our Population Health Career Lattice (PHCL) aims to bridge this gap, making a career in health care more than just possible. We provide clear, structured pathways to advance from entry-level to experienced community health worker (CHW), care management (CM) and clinical health coaching (CHC) roles, equipping you with essential skills to support your communities and advance your careers. We provide tuition scholarships, course completion stipends, social supports and funds to mitigate emergency childcare, health, transportation and housing challenges you may encounter.

The Population Health Career Lattice (PHCL) program is designed to support individuals who are passionate about making a difference in their communities. Eligible participants include:
- Youth aging out of foster care
- Graduates of alternative schools entering the workforce
- Young adults (ages 17-24) living at or below the poverty level
- Individuals committed to serving underserved populations in Iowa, Colorado and Missouri

Engagement Stipends
In addition to tuition scholarship, enrollees may earn a stipend dispensed at various phases of training completion.Â

Community Health Worker Professional Skills Training
As part of PHCL, we offer a comprehensive Community Health Worker Professional Skills Training program. This skills-based training includes five on-demand courses, 4 virtual live sessions, and quizzes and assignments covering the following topics:
- Introduction to Community Health Work: The Big Picture
- Core Competencies for Providing Direct Services
- Enhancing Client Interactions
- Enhancing Professional Skills
- Applying Core Competencies to Key Health Issues
Essentials of Care Management Training
One of our PHCL offerings, Essentials of Care Management online training allows care managers to acquire skills for integrating into the care team, establishing and implementing care management processes, tools, and resources, and educating and activating patients to adequately address their health care needs. This is a comprehensive learning path covering the following topics:
- Foundations of Care Management
- Episodic and Longitudinal Care Management
- Patient Engagement and Relationship Building
Clinical Health Coach® Training
- Effective health coaching skills that result in a patient-centric environment.
- Clinical assessments and tools that support the guided-care process and patient-selected goals for behavior change (especially for those with chronic conditions).
- Population health strategies to identify, stratify, track, and measure select interventions for empaneled patients.
- Effective health care communication to improve health literacy.
