What is the Population Health Career Lattice Program?
Health care faces a dramatic workforce shortage, while young adults entering the workforce are met with financial, societal, and situational barriers preventing them from reaching their full potential.
We want to change that.
HealthTeamWorks’ Population Health Career Lattice (PHCL) program focuses on providing clear, structured pathways to advance from entry-level to experienced community health worker (CHW), care management (CM) and clinical health coaching (CHC) roles, equipping enrollees with essential skills to support their communities and advance their careers.
Our Population Health Career Lattice enhances established training programs and offers additional resources to qualified trainees, including tuition scholarships, course completion stipends, social needs assessments and supports/funds to mitigate emergency childcare, health, transportation and housing challenges trainees may encounter.
But to provide the best possible supports to our trainees, we need organizations like you.

What would that look like for your organization? ​
Diversity of needs
There are many different needs that could prevent someone from being able to access further education, and as such, there are many different types of social supports created to help. This is to say, no two organizations will support the Population Health Career Lattice the exact same.
What we can tell you:
This won’t look too different from what you’re already doing! On our side, PHCL trainees are invited to complete a five question social needs assessment built on the HealthLeads tool. Based on identified unmet needs and consent, we will reach out to attempt warm handoff with your team for services you may be able to provide or coordinate. We request only confirmation that our trainees have made follow-up contact after our introduction.
Aside from just supporting the cause...
As an added element of joining the PHCL support network, your organization can expect:
- Periodic updates on PHCL impacts
- Industry news updates and a digest fof opportunities specific for organizations employing CHWs, CMs, and CHCs
- Name/Logo inclusion in our PHCL materials, such as our website, handouts, etc
