Colorado Patient-Centered Medical Home Residency Program

The Colorado Residency Patient-Centered Medical Home (PCMH) Program focused on redesigning residency curricula to train physicians in the core competencies of PCMH care delivery. HealthTeamWorks, in partnership with the University of Colorado School of Medicine and the Colorado Association of Family Medicine Residencies, provided practice transformation services to support this initiative.
