An AIM Statement Should Include:
- A Specific, Measurable, Attainable, Realistic, Timely (SMART) goal
- Where and when will the change occur
- What will be done
- The start and end of the process
- Who will benefit from this improvement
- The impact of meeting the goal
When Creating Your AIM Statement :
- Describe the system or process you are aiming to improve, increase, or decrease​
- Define a measure of improvement, e.g. a count or percentage
- Define the location where the change will occur
- Identify a date for achievement of the goal
- Identify a starting point for the process
- Define the end of the process
- List the benefits you expect to see when the process is complete
- List the impact of meeting the goal
Example of an AIM Statement:
“We aim to increase depression screening for patients aged 12 and older with a follow-up plan, as needed by 4 % at the Primary Care Clinic Downtown by December 31, 2020. The process begins with the rooming of the patient and ends when the physician leaves the exam room. By working with the process, we expect to identify patients who need additional resources, and improve the MIPS measure. It is important to work on this now because patients will get the care they need, and the MIPS reimbursement will increase.”
