Hanna Moffett has over 10 years of experience in health care and a strong background in telecommunications with Nokia Corporation in Finland, Germany, and the U.S., where she gained expertise in marketing, communications, project management, and collaborating on multicultural, cross-functional projects.
She joined HealthTeamWorks in 2015 as a Program Coordinator, serving as a member of the Regional Learning Faculty team in Colorado for the Comprehensive Primary Care (CPC) Initiative and later supporting the Comprehensive Primary Care Plus (CPC+) Colorado and Nebraska Regional Learning Network. In these roles, she worked closely with the client, Practice Facilitators, and Program Managers to oversee deliverables and timelines and coordinate program communications and materials.
Now serving as Program Manager, Ms. Moffett manages several projects designed to strengthen care teams, improve quality and safety, enhance the care experience, and advance success in value-based care. These include overseeing deliverables and timelines for past federal value-based payment initiatives and leading current state-level projects that provide technical assistance, coaching, and Behavioral Health Integration, as well as workforce development programs that deliver customized training to build capacity and sustainability. In addition, she oversees HealthTeamWorks’ marketing and office operations and directs the organization’s newsletter, guiding strategy, content, and delivery.
Her background with large-scale national healthcare initiatives has given her a strong understanding of the project requirements needed to support value-based payment models and the role of facilitation teams in helping practices meet those demands. Her expertise spans project management, process improvement, stakeholder engagement, event management, communication and content development, and operational management.
Fluent in English, Finnish, and German, she draws on her international experience—having lived and worked in Finland, Germany, and the U.S.—to bring a global perspective that strengthens her ability to collaborate effectively with diverse teams and clients. Born in Finland, she began her education in Swaziland and graduated from Helsinki Secretarial College in Finland.
