Integrated Care Delivery Support – We partner with organizations to integrate behavioral health into their care models. Through programs like the 1302 Behavioral Health Integration Program, we provide guidance and technical assistance to assess each clinic’s unique needs, develop customized implementation plans, and offer ongoing, practice-level support. We also facilitate peer-to-peer learning opportunities, enabling clinics to share real-time challenges and solutions as they work toward sustainable, evidence-based models of behavioral health integration.
Driving Population Health Innovation – We engage in advocacy and collaborative initiatives to strengthen high-quality, team-based care. As a member of the Primary Care Collaborative, we contribute to national efforts that advance and elevate primary care delivery. Regionally, we serve as the Regional Health Connector for Region 16 in Colorado, helping link clinical care with community-based resources. We participate in community-focused initiatives such as the Kansas City Community Benefit Coalition and the KC Health Equity Learning and Action Network, working alongside partners to address social drivers of health and promote cross-sector collaboration.
Quality & Performance Improvement – We work alongside organizations to drive improvement across clinics, networks, and health systems. Our support spans practice transformation and alignment through facilitation, technical assistance, and strategic coaching—helping teams implement sustainable changes, meet quality goals, and improve outcomes. We also provide consultation for PCMH recognition and accreditation, supporting organizations in building sustainable, patient-centered systems of care. Through tailored coaching, team training, and data-driven strategies, we help strengthen infrastructure and sustain long-term success.
Strengthening Organizational Capacity –We specialize in helping organizations grow from the inside out—building internal alignment, enhancing team effectiveness, and driving sustainable change. Our support includes strategic planning, change management, competency development, and retreat facilitation—tailored to your goals and challenges. Whether improving workflows, mitigating workforce burnout, or preparing for value-based care, we provide practical strategies that build resilience and support long-term success in today’s evolving health care landscape.
Workforce Development & Training Pathways – Through programs like the Population Health Career Lattice (PHCL), we provide clear, supported pathways into roles such as community health worker, care manager, and clinical health coach—especially for individuals who may face barriers to education or employment. Alongside PHCL, we provide workforce development training for organizations and individuals in Care Management, Clinical Health Coach®, Community Health Worker, Quality Improvement Facilitation, and Culturally Relevant and Affirming Care.
