HealthTeamWorks remains steadfast in supporting practices in delivering whole-person care by equipping them with the tools and strategies necessary to meet the varied needs of their communities. This includes expanding our support for behavioral health integration and addressing health-related social needs.

Health care workforce sustainability is central to our mission. We will continue to invest in the training and development of Community Health Workers, Care Managers, and Clinical Health Coaches.
