Empowering Teams to Improve Health Across Communities

In today’s complex and often fragmented health care system, helping people maintain or regain their health requires more than medical treatment—it takes connection, communication, and coordination. At HealthTeamWorks, we believe the key to improving population health lies in empowering those who support patients across every stage of care—from clinical teams to community-based workers.

Whether someone is managing a chronic condition, navigating a care transition, or trying to prevent illness altogether, having access to the right support at the right time can make all the difference. That’s why we’ve developed targeted training programs that equip health care professionals with the knowledge and practical skills to meet patients where they are—clinically, emotionally, and socially.

Our work focuses on building the capacity of the health care workforce to engage individuals more meaningfully, address the root causes of poor health, and drive better outcomes across communities. Through evidence-based training in health coaching, care management, and community health work, we are helping to transform health care from the ground up—ensuring it is not only effective, but also compassionate, culturally responsive, and accessible to all.

Below, we explore how our three signature training programs are making a difference—and why they’re essential to the future of health care.

Clinical Health Coach® Training Program – Promoting Behavior Change

The Clinical Health Coach® Training Program is designed to empower care teams with the skills to support meaningful behavior change in patients. Offered in both Online and v-Fusion formats, the program trains participants to integrate motivational interviewing, collaborative goal setting, and structured self‑management support into their daily workflows—whether during brief visits, telehealth check-ins, or ongoing care relationships.

Health coaching has been shown to improve a wide range of outcomes. Patients working with trained health coaches experience better medication adherence for chronic conditions like type 2 diabetes, improved cholesterol levels, and greater self-management confidence. By embedding coaching skills into clinical practice, providers foster trust, support patient autonomy, and enhance the quality of every interaction. These tools are essential not just for individual outcomes, but for advancing value-based care and long-term population health.

Community Health Worker Professional Skills – Engaging Hard-to-Reach Populations

Our Community Health Worker (CHW) Professional Skills training equips CHWs to serve as vital connectors between the health care system and the communities they serve. Through training in culturally responsive communication, patient advocacy, and health education, CHWs learn how to bridge gaps in care, especially for vulnerable or underserved populations.

CHWs are proven to drive significant impact. In U.S.–Mexico border communities, for example, cancer screening rates rose by 35% when CHWs were involved. Their ability to build trust, navigate cultural nuances, and provide tailored support helps individuals overcome barriers to care and remain engaged with health services. Our training strengthens CHWs’ confidence, expands their impact, and amplifies their role in reducing disparities and advancing health equity.

Care Management Training Program – Coordinated, Proactive Care

The Care Management Training Program is designed to support care coordinators and managers in building proactive, patient-centered care plans. Participants gain skills in managing complex patient needs, facilitating care transitions, coordinating across systems, and addressing the social determinants of health.

Structured care management is essential to reducing unnecessary hospitalizations, controlling chronic conditions, and improving overall system performance. With training, care managers are able to align services effectively, ensure timely interventions, and provide high-touch support to those who need it most. These professionals play a central role in making care seamless, sustainable, and responsive to the needs of the whole person.

Why Our Work Is More Relevant than Ever

As health care continues to shift toward value-based care, organizations need a skilled, agile workforce capable of addressing both clinical and non-clinical drivers of health. Our training programs deliver that capacity:

  • Clinical Health Coaches empower patients to take ownership of their health.
  • Community Health Workers extend care beyond the clinic into homes and neighborhoods.
  • Care Managers create clear, coordinated care pathways for high-risk patients.

Together, these roles form a powerful support system—one that not only treats illness, but also promotes lasting health, resilience, and well-being.
