Our Approach
Unlike our online Essentials of Care Management training, we deliver the Intermediate Care Management learning path as live virtual courses. We focus on the customized learning and coaching support needed to apply and sustain care management. Our facilitators help experienced care managers with:

- To identify community resources & analyze data to identify social needs in populations & develop community resource toolkits
- To identify the impacts of low literacy on health outcomes & the skills & tools to address them
- To integrate care management into the care team
- To build practice relationships & effective partnerships with providers
- To develop healthy boundaries
- To collaborate with community partners & develop care agreements
- To assess the effectiveness of care management programs & communicate success
