HealthTeamWorks promotes integrated care delivery to address unmet needs.

Behavioral Health Integration

Addressing Unmet Needs

We address the crisis of unmet behavioral health needs and the resulting outcome disparities by increasing
behavioral health integration in primary care.

There is no one-size-fits-all solution when it comes to integration of behavioral health care. Successful models must vary based on resources and population needs.

As integrated care goes beyond providing behavioral health services, all facets of a practice must reflect the values of whole-person health, collaborative care, and a shared responsibility for outcomes. This includes workflows, policies, business models, culture, and norms.

Our team works with organizations to overcome the challenges of integrating the foundations of prevention, identification, and treatment of behavioral health into their practice.

We also provide training and technical assistance on effective leadership strategies to assess and mitigate burnout by providing support to practice care teams.
