HealthTeamWorks is seeking a Marketing Specialist to lead the process of all outbound and inbound marketing activities, including content development and optimization, advertising through social media, and event planning. The Marketing Specialist will be expected to manage creative projects from conception to completion, so graphic design experience is also a must-have. Ideally, our rockstar Marketing Specialist will have experience in healthcare, with some knowledge and/or experience in healthcare transformation and performance and quality improvement. Healthcare Learning Faculty

The Healthcare Learning Faculty exhibits courage, innovation, flexibility and a “can do” attitude while managing, planning and delivering concurrent virtual “action groups” (multi-session webinars on a focused topic developed to be interactive and support primary care practices in conducting small tests of change) as part of the Comprehensive Primary Care Plus model. The virtual participants are comprised of medical practice representatives dedicated to a particular performance improvement measure or activity. The Healthcare Learning Faculty is accountable for content development and virtual facilitation of all assigned action groups, including convening groups in coordination with regional learning network support, and other support tasks to achieve contract deliverables as needed.

Practice Improvement Facilitator

As a member of the HealthTeamWorks Practice Transformation Team, the Practice Facilitator (PF) manages a caseload of practices involved in one or more of the department’s improvement activity programs.  The PF utilizes an evidence-based approach to facilitating a multi-disciplinary quality improvement (QI) team working directly within practices and within the organization. The PF maintains accountability for the baseline assessment, ongoing facilitation of practice improvement methodologies & measure reporting as well as ongoing assessment through internal quality improvement activities of initiative and individual practice progress toward desired transformational change.

Clinical Health Information Technology Advisor

The Clinical Health Information Technology Advisor (CHITA) supports practice transformation objectives by coaching designated clinical practices in the optimal use of available technology and data.  The CHITA will help optimize data input and extraction, data integrity, clinical quality measure reporting, data analysis, and application of data insights to guide quality improvement activities in the clinical practice setting. A successful CHITA will collaborate closely with the healthcare practice’s internal HIT personnel, HealthTeamWorks Practice Facilitators, Community Health Connectors, and the statewide learning network of other CHITAs.
